New Patient Forms
If you’re a new patient, we will email you a link to our portal to do your online paperwork for ease or if would like to fill out paperwork before your visit, feel free to download those forms here. If you are not able to fill them out beforehand, please arrive fifteen minutes early to fill them out in the office. We’re looking forward to seeing you!
Insurance & Financial Policy
We collect payment at time of visit. If you have insurance with a provider we are in network with, we will file your visit and notify you if any balance is remaining. We are in network with:
BLUE CROSS BLUESHIELD, CIGNA, AETNA, STATE HEALTH PLAN, AND MEDCOST. We are able to file HUMANA for State Health Plan retirees.
We are Non-Participating with Medicare. While we can still file with Medicare for you to be reimbursed directly for a portion of your visit, please note that Medicare does NOT pay for any exams or therapies performed by a chiropractor. Medicare will reimburse for spinal manipulation ONLY (subject to deductible status), which is only a portion of your visit.
If you do not have health insurance or we are out of network with your plan, we do provide self pay rates. Ask us for details.
Cancellation Policy
We understand that schedule adjustments are necessary sometimes and we respectfully request at least a 24 hour notice for cancellations. If you cannot make your scheduled appointment, please call us at 919-213-0881.
